What is a Geriatric Care Manager?

	Geriatric Care Managers (GCMs) are individuals with expertise in dealing with issues regarding health, disability, geriatrics, home and community-based care and services, assisted living facilities, nursing homes and local, state and federal assistance and benefit programs.
	The GCM performs an assessment of the older person, their current living environment and circumstances and their support network to develop a long-term plan of care that will maintain the independence and safety of the older person, as well as provide support to their caregivers.
